Add tablix header separator line

Adds two narrow spacing rows and a separator beneath the final tablix header row. An optional hidden row catches labels inadvertently inserted into the header by a designer, keeping them out of the visible output. Existing separator and catch-all rows are recognised rather than duplicated.
